Kindly confirm vaccination appointment two days prior to your visit.
Please carry this card for every visit.
Disclaimers
- Packages are for vaccines from birth till 1 year and DO NOT include any medication / investigation charges.
- Other vaccines / interchanging of vaccines will be charged additionally.
- Vaccines may be replaced with a similar brand only in case of unavailability.
- In case of delay in vaccination schedule, the doctor’s decision regarding vaccination will be final and binding.
- No refund will be applicable for missed / delayed vaccinations.
- Vaccines will not be administered without this card.
- Card may be replaced in case of damage / loss at an additional cost of Rs.500/.
- No refund / cancellation of the package is permitted.
- The decisions of Hospital and Management are final and binding.
